The verbatim official rule from Japan's Ministry of Health, Labour and Welfare is shown below in the original Japanese.
Why this spec matters
An Occupational Therapist (OT) in Japan is a national license holder who supports people with physical, mental, or developmental disabilities in recovering daily-living function through "occupations" — activities such as eating, dressing, bathing, fine-motor handicrafts, and vocational training for return to work. Unlike Physical Therapists (PT), whose remit by law is the recovery of basic motor functions (standing, walking), OTs cover both physical and mental/developmental domains. The OT and PT national exams are held on the same day each January at the same venues, and share an identical photo spec of 60mm tall × 40mm wide. This is the unified spec across Japan medical national exams — nurse, midwife, public health nurse, pharmacist, dentist, radiological technologist — and is one size larger than My Number or driver's license photos (35×45mm), so the "ID / driver's license" size at a convenience-store photo booth will be rejected. The OT exam also has a unique procedure: write the date taken and full name on the back of the printed photo, then glue it to the official "exam photo mounting sheet" issued by MHLW or the exam administration office.
